What the form will ask

  • Your training organisation, course and year of training
  • Your UKCP trainee membership number, if you have one
  • Your supervised client hours and personal therapy hours
  • Your primary supervisor and your personal therapist, and the body each is registered with
  • DBS and indemnity insurance status
  • Your availability
  • A CV and a recent photo
